    All PI-CME participants have signed the following attestation statement reflecting their commitment to completing this activity:

    "Through participation in this Patient-Centered Quality Pain Care Initiative, in my practice setting I commit to reducing harm and other adverse events related to Chronic Opioid Therapy while maintaining access to the appropriate use of pain analgesics.

    I also agree to:

    • conduct an individual self-assessment at the start of each PI CME Module;
    • develop a self-directed improvement plan based on the core practice standards set forth in this Initiative;
    • participate in monthly hour-long web-based education sessions;
    • complete practice change surveys at 6, 12, and 18 months ; and to
    • collect and report practice and patient data at appropriate intervals.

    At the conclusion of each PI-CME Module, I agree to complete an assessment to measure my progress/practice improvement towards attaining the goals I put forward in my individual improvement plan."
